If You Already Have TennCare

  • Contact Your Managed Care Organization (MCO): Call the number listed on your TennCare card to inquire about eligibility and services.

If You Do Not Have TennCare

  • Apply Online: Use the TennCare Connect portal to submit your application.
  • Apply by Phone: Call TennCare Connect at 1-855-259-0701 for assistance.
  • Apply by Mail or Fax: Download and complete the paper application, then mail it to:

For Long-Term Services & Supports (LTSS) or CHOICES
